Physical Therapist Job Description/Requirements:
- Evaluation/Treatment of Patients in their home. Complete evaluation and treatment of patient in compliance within the Physical Therapy Professional Scope of Practice.
- Assist in evaluating the level of function and in developing the plan of care.
- Evaluate the home environment and makes appropriate recommendations.
- Provides Continuity of Care: Ensure appropriate planning for patient discharge/reassessment and referral to other services according to agency policy.
- Communicates: Communicate with care coordinator and other health care professionals caring for the patient to ensure uninterrupted patient care.
- This is an independent contractor position and the pay is $85-115 per visit. Potential to make $150,000- 180,000 yearly.
- We are hiring in multiple location across Southern California (Los Angeles County, San Bernardino County, Riverside County, Orange County)
Minimum Required Qualifications:
- Current CA Physical Therapy (PT) License
- Valid California Driver's License/ with transportation insurance

Pay: $85.00 - $115.00 per visit
